Disabled people - independence at home
CONTACT REFERRAL TEAM
The Contact Referral Team is part of the Community Health & Care Partnership (CHCP) and offers an assessment and support service to people who require assistance to manage in their own homes. This sort of help is assistance with managing their money, paying bills, claiming benefits, advice concerning neighbour disputes, helping to make appointments with professionals, assisting with shopping etc. It does not provide help with personal care but is practical in nature.

LEARNING DISABILITY INDEPENDENCE TEAM
The Learning Disability Independence Team provides support to vulnerable adults, e.g. placements for adults with learning disabilities and housing support to adults with a range of needs.
Adult placements offer accommodation and support in a specially recruited and trained carer's home.
Housing support (including personal care where required) is provided to adults who have their own accommodation and who need support to continue to live at home. The housing support team can also help adults who have been assessed as needing to move to their own accommodation by supporting them to establish themselves in their new home.
The service is available to anyone who has a learning disability, is living in West Lothian and is aged over 16 years.
