Application Form for Registration of a  Food Business

A food business which is operating within West Lothian should be registered with Environmental Health and Trading Standards 28 days prior to opening.  The downloadable pdf should be completed and sent to Environmental Health and Trading Standards, County Buildings, High Street, Linlithgow. EH49 7EZ or emailed to environmentalhealth@westlothian.gov.uk.
